- Dr. Afshar is a contracted provider for Blue Cross/Blue Shield and NH Medicaid. If you have other insurance coverage, you may be able to use what is generally called a Single Case Approval by your insurance company. You will need to call them and ask them for this approval well ahead of your first appointment.
- If you do not have insurance coverage, we offer a sliding-fee scale based on income and level of need. There are also package products with retainer payments that offer discounts and savings. Please call Dr. Afshar to discuss your financial needs.
- All payments including co-pays are due at the time of service.
- Cancellations with less than 24 hours are charged to you (not your insurance company) at the rate of $125 per cancelled session.
- No future appointments will be made if there outstanding balance on account.
- Dr. Afshar accepts cash and checks only.
- Not all services are covered by insurance policies.
